I don't know how you'd classify it in terms of genre, but Zombie by Joyce Carol Oates is hands-down the most disturbing novel I've ever read. It's one of those books that you can't put down, but it's really hard to recommend it to someone else unless they realize what they're in for.

I just looked it up, its sounds right up my street! I read the author based the main character on Dahmer; I find him the most fascinating serial killer so this is going on the list! Thank you :)

Non-fiction but "The Man from the Train" by Bill James. An account of someone who was roaming about in the early 1900's breaking into homes and murdering families with an axe. He may been responsible for as many as one hundred deaths including many children. Some very chilling details including how he covered up the mirrors inside the homes and staged many of the bodies. The book is written in a kind of folksy style that is somewhat repetitive but it's a very disturbing story.

The short story "Guts" from Chuck Palahniuk's collection Haunted. It's hard to forget once you've read it.
Chuck.

I have tried really really hard to like Chuck because all the cool kids do. I just can't. I have read Guts. It's available online for anyone who wants to read it. It's short. And it is disturbing.
