I am of Hispanic descent and its been years since I have heard of El Cuco and it is not just made up for the Outsiders. El Cuco is actually used in Many of the different Spanish dialects Cuban, Mexican, Spanish, Puerto Rican and I assure you the South Americans probably have their versions of El Cuco. I am hard of hearing so I always have subtitles on so if I don't pick it up by hearing it I can read it and still get the meaning if and in the event my hearing fails me. Now I have seen this in the past and it is where someone who is not Hispanic tries to say something that is predominately Spanish and winds up turning their meaning into one serious debacle. Glory who is trying to make sense of the death of her husband blows a gasket when Holly tries to introduce the theory that El Cuco is involved and in reading the subtitles She refers to El Cuco as El cuck·oo which is referring to a cuckoo clock. I found this hilarious and wondered if this was part of Stephen Kings Genius or if there is an actual name for this comedy of errors !!!

I'm sure it was Stephen having the character react with disbelief that Holly was cuckoo like in crazy. Not as in cuckoo clock. The character was angry and she was being insulting to Holly on purpose.
