He can save her. He can turn back. To lose a few days. Back to Tull. To save the child. Continue your journey to the tower with her. The last life saved from a past life. The first life saved on a new journey to the Tower. It is symbolic.

Maybe if he would save this little life, preserve this life on the way to the tower, enter the Tower, carrying her in his arms. Maybe he'll get some peace.
This could be a new story about gunslinger.

I think I get what he's asking: Roland shot a lot of people in Tull (supposedly everyone in the town), but Alexander wonders what happened to the Kennerly baby. (Kennerly had four daughters, one full grown, two half-grown and a baby). He suggests that it's unlikely Roland shot the baby, so what happened to it? A legit question, but I'm not sure there's a satisfactory answer. Its fate is never revealed, but it probably wouldn't have survived long if everyone else in town was dead.
Yes, this scene is one of those brutal revelations about the gunslinger that makes him hard to like, like the scene under the mountain. These are the views that show us a side to roland we may not be as comfortable with. You understand that the preacher (I believe she called herself a prophet of BuffaloStar) has called upon all of the people of the town to rise against him, but what we will not understand till later is why, ( the world has moved on), and how this feels to the gunslinger- in his previous life he was one who might have protected these people, not only from the death he brought them but from bad leaders, and even the phenomenon of the world moving on, in his quest for the Tower. that can’t feel good. Of course It also shows his skills as a walking death machine, and highlights how bad life is for the people of inworld (as Lud also does) I don’t know if that’s as well reasoned as my comments usually are but im a little tired.
