Good choice, for me I'd say Annie Wilkes. . .I think she was based on a real nurse.
I know some mean crazy azz women like that. I wouldn't try to fight her . . .I'd just shoot her and be done with it. . .I'd unload too!
The bad guy (the old man) from Black House was a pretty scary fella. Can't remember his name, but he was a major creep.
The Space Cowboy from Gerald's Game. Creeps me out whenever I think about him.
I guess if I would have read the thread before posting, I would have realized that he was called The Fisherman...

I think Gage in Pet Sematary scared me the most, but I would give an honorable mention to Rachel's sister. I had nightmares about her.
Ah Zelda! You know they got a man to play her in the film!
I thought Zelda looked a little on the masculine side.
Now I know why!
Zelda in the film is one of the weirdest looking characters ever, would easily scare the crap out of kids, and even leaves me feeling a bit uneasy.
